It should already be included with your joomla install.

In the Module Manager, click on New, and choose Login as the module type. I called mine "Mobile Login".

Hi,

I am using Joomla 2.5.2 and Mob Joomla 1.0.1

Still getting the 404 when the login module redirects me to index.php?device=xhtml

Have you tried playing with Login redirection settings? 
At your Mobile login module try changing the redirection page after login. 
Try the same for your regular login module. I have set the Joomla login redirection page to my home page.

A good idead, but I tried that. To access my test site, you have to go through a specific port, and I thought that the Login redirect might not be pointing to the correct port. But that doesn't seem to be the case. I will just have to enable it on the live site and hope for the best . ~_~

Joomla! 2.5.2. Uninstalled mobile Joomla then installed ver 1.0.1. Using default Joomla login module in mj_all_footer position.

Login with mobile_smartphone template using Android device works.
Login with mobile_smartphone template using iPhone device works.
Attempt with either device to login using mobile_iphone template gives me 404- Component not found.

Fixed in repo, will be released in MJ1.0.3. You can apply following patch to templates/mobile_iphone/html/mod_login/default.php: github.com/mobilejoomla/mobilejoomla/commit/5535043e4e6025b901b75d8645c19dc1eab6d481
